Facilities in Pennsylvania follow a regulation from the Pennsylvania Department of Health regarding what is needed on a physical.
This is a state requirement, not an IntelyCare requirement, so we are unable to waive it. If you have a physical uploaded from working in a different state, it will not work for Pennsylvania shifts unless it contains all the correct information.
What needs to be on my physical?
Pennsylvania physicals must meet all the requirements below:
- The document must be signed by a healthcare practitioner.
- It will need to contain a statement that you are “free from communicable diseases”.
- The document will need to state that you can perform your work duties without restriction.

The Pennsylvania version of the IntelyCare Physical Form, which is attached to this article, contains everything that is needed for a valid physical in Pennsylvania.
